Keto Rash Is the Latest Side Effect of the Low-Carb Diet—Here’s How to Treat It

When it comes to the keto diet, you either love it or you hate it. But even the biggest keto devotees admit that the high-fat, low-carb plan can have some unwanted side effects (think stinky breath, diarrhea, and flu-like symptoms).

Lately, we’ve been hearing about a new side effect to add to the diet’s rap sheet, and it’s called keto rash. Here’s everything we know about this red, itchy body rash some keto followers wind up with, including the main symptoms and most effective treatments.

Keto rash symptoms

Keto rash, officially known as prurigo pigmentosa, is a rare form of dermatitis, or skin inflammation. It appears as raised, red, itchy bumps on the skin that take on a web-like appearance. The rash may resemble contact dermatitis, or an allergic reaction, but it can be distinguished by the brownish discoloration of the skin that remains after the inflammation subsides. Keto rash can show up anywhere on the body, but it’s more common on the chest, back, and neck.

What about the keto diet causes it?

Experts aren’t sure, but there are a few theories. “Because the keto diet is 75% to 90% fat, people may be eating large quantities of common allergens, including dairy, eggs, and seafood,” Cynthia Sass, RD, Health contributing nutrition editor, tells Health. “These foods can also be triggers of food sensitivities, which are non-allergic reactions that cause the immune system to release inflammatory compounds which in turn lead to reactions, including skin inflammation and joint pain.”

Another possible cause, according to Sass, is that the diet’s lack of fiber and plant-based foods alters gut bacteria in a way that increases the production of inflammatory compounds. “A high intake of red meat, another common pattern in a keto diet, can do the same,” she adds. Yet another theory has it that the keto diet exposes the body to a larger chemical load, thanks to the emphasis it places on meat and dairy.

“In my opinion, keto rash is a sign that your body is not responding well to the keto diet,” she says.

How to treat keto rash

In many cases, keto rash can be eased by adding carbs back into your diet. But if you’re not ready to give up on keto, Sass advises that you “prioritize plant-based monounsaturated fats, like extra virgin olive oil and avocado, over cream and butter.” She also suggests eating at least five cups of veggies per day, adding back a few servings of fresh fruit, limiting or eliminating red meat, and consuming small portions of nutrient- and fiber-rich whole carbs, such as lentils, quinoa, potatoes, and oats.

Debra Jaliman, MD, New York City-based dermatologist, tells Health that reintroducing foods like fruits and veggies to your diet will also provide your body with essential vitamins your skin may have been missing while on keto. “The best diet for the skin is one that’s varied with antioxidants,” Dr. Jaliman says. She also recommends eating a moderate amount of healthy omega fats, which can be found in foods like salmon, flaxseeds, and soybeans.

If your keto rash doesn’t go away by making these tweaks to your diet, speak with your doctor. Some antibiotics, such as minocycline and doxycycline, can treat prurigo pigmentosa. Your doctor can also help you get to the bottom of what caused the rash in the first place and made it noticeable when you started keto.

  • This may be because drastic dietary changes can cause a pH imbalance in the vagina, according to Lisa De Fazio, a registered dietitian nutritionist.
  • A pH imbalance can cause “bad” bacteria to grow, causing problems like irritation and odors in the vaginal area.

The keto diet is having a moment right now. Originally developed to treat epilepsy patients who were not responsive to traditional medicine, the trending, low-carb diet aims to put the body into ketosis, a state in which it burns fat, not carbs.

The keto diet isn’t without side effects, though. According to Lisa De Fazio, a registered dietitian nutritionist, the keto diet can put followers at risk for kidney problems, dehydration, the “keto flu,” and more. That’s not all, though. Some Reddit users are talking about one strange side effect known as “keto crotch.”

Keto crotch refers to a change in vaginal odor

Since the keto diet calls for a mix of high-fat, low-carb foods with moderate proteins, it can be an adjustment for your body to get used to — and it may cause some funky side effects like a change in your vaginal odor. 

Though there is no research done on the topic, the medical phenomenon may happen when you drastically change your diet by incorporating large amounts of certain macronutrients like fat and protein, according to De Fazio. 

“Foods change the pH of the body. When this happens, the body will emit certain odors,” De Fazio told INSIDER. “The keto diet [may] change your vaginal pH, which alters your vaginal odor — and it may not smell like roses.” 

So, why does all of this matter? Well, a pH imbalance can bring upon “bad” bacteria growth.

Additionally, De Fazio said people with a pH imbalance can develop irritation, odor, and infections such as bacterial vaginosis— the most common vaginal infection for menstruating people.

To better understand keto crotch, it’s important to know how pH works

First of all, pH stands for potential of Hydrogen. It uses a number scale of zero to 14 to measure the amount of acid or alkaline of a substance.

According to De Fazio, a pH of less than seven is considered acidic and a pH of more than seven is basic. 

“The pH-balance is an important factor in maintaining vaginal health,” she said. “A high-fat diet, particularly saturated fat like on the keto diet, increases vaginal pH, and therefore increasing the risk of bacterial vaginosis.”

A vagina should have a pH level between per De Fazio. This means that it is moderately acidic. De Fazio, however, is quick to point out that what constitutes a normal pH level can vary slightly based on your stage of life.

“During your reproductive years, ages 15 to 49, your vaginal pH should be below or equal to . But, before menstruation and after menopause, a healthy pH tends to be higher than ,” said De Fazio. “An acidic vaginal environment is protective. It creates a barrier that prevents unhealthy bacteria and yeast from multiplying too quickly and causing infection.”

A high vaginal pH level — above 4.5 — provides the perfect environment for unhealthy bacteria to grow. It also puts you at risk for certain infections, according to De Fazio. Luckily, keto crotch can be treated and it may even go away on its own, she said. 

“It may go away, otherwise you should see your OB/GYN for treatment,” said De Fazio. “Your doctor may advise you to go back to a balanced diet. Any extreme changes to the body or pH is not a good idea.”

Loaded Mock Keto Potato Salad | Peace Love and Low Carb

Ingredients

  • 1 large head cauliflower, trimmed and cut into small florets
  • 2 tablespoons olive oil, butter or ghee
  • Sea salt and ground black pepper
  • 5 slices bacon, cooked crisp and chopped
  • 3 green onions, chopped
  • 2 large hardboiled eggs, rough chopped
  • 3/4 cup shredded sharp cheddar cheese
  • 1/3 cup mayonnaise, more for a creamier salad
  • 1/3 cup sour cream
  • 3 tablespoons chopped dill pickles
  • 1 tablespoon Dijon mustard
  • ½ teaspoon garlic powder, to taste
  • ¼ teaspoon paprika

Instructions

  1. Preheat the oven to 400° F.
  2. Spread the cauliflower florets in a single layer across a rimmed baking sheet. Drizzle the olive oil over top and season generously with salt and pepper.
  3. Roast for 25 minutes or until tender and golden brown. Remove from the oven and allow to cool.
  4. Transfer the cauliflower to a large mixing bowl. To the bowl add the bacon, green onions, eggs, cheese, mayonnaise, sour cream, pickles, Dijon mustard, garlic powder and paprika. Mix until all ingredients are well combined.
  5. Refrigerate for one hour, up to overnight before serving.

KETO SUGAR FREE BUTTERMILK SYRUP

INGREDIENTS

  • ¾ cup unsalted butter, grass-fed if possible
  • 5 tablespoons stevia or ⅔ cup granulated erythritol
  • ½ cup heavy whipping cream, pasture-raised if possible
  • ¼ cup water
  • ¾ teaspoon apple cider vinegar
  • Pinch of pink Himalayan salt
  • 1 teaspoon baking soda
  • 1 teaspoon vanilla

INSTRUCTIONS

  1. In a large, high-sided pan, melt the butter over medium heat. Be sure your pan is large enough to accomodate the syrup as it will foam up when baking soda is added.
  2. Add water and either stevia or erythritol. Whisk until the sweetener is completely dissolved.
  3. Whisk in cream, vinegar, and salt.
  4. Bring to a boil while gently stirring.
  5. Remove from heat and gently whisk in vanilla and baking soda.The syrup will begin to foam up at this point.
  6. When the foam has receded carefully pour the syrup into a syrup pitcher and serve! Enjoy!

NOTES

If you use erythritol in this pancake syrup recipe, it will crystallize after cooling. Just reheat and stir till the sweetener is dissolved into the syrup again and serve!

  • 4 egg yolks

Instructions

  1. In a stainless steel mixing bowl, whisk the egg yolks and lemon juice together. The mixture should get thicker and increase in volume.
  2. Heat a saucepan with 1 to 2 inches of water in it over medium heat until the water is simmering. Lower the heat to medium low. Place the bowl over top of the saucepan, making sure that the water is not touching the bottom of the bowl or else the eggs will begin to scramble. Continue whisking rapidly.
  3. Little by little, whisk in the melted butter until the sauce has thickened and is light and fluffy.
  4. Remove from the heat and gently whisk in the hot sauce, cayenne pepper and sea salt.

keto rash Keto flu, keto breath, keto crotch… What’s next? Unfortunately, new side effects of the keto diet continue to emerge. People are now breaking out in full-body, painful rashes after going on the extremely low-carb diet. And perhaps the worst part is that this “keto rash” can leave brown marks on the surface of the skin that stick around for life.

One of the most popular diet trends of this decade, the idea behind the keto diet is that restricting carb intake to an extremely low number while eating large amounts of fats will trick the body into entering ketosis, a state in which the body burns fats for fuel. Ketosis is difficult to maintain, but once it is achieved, the body can react in strange (and in this case painful) ways.

If you search “keto rash” on Reddit, you’ll find dozens of dieters begging for help with the condition, sharing anecdotes of excruciating itching and discomfort. The rash tends to start near the chest and spread, reaching every part of the body, including the scalp.

“I noticed a rash across my left boob within 3 days,” one Reddit user recalled. “By day 14 (Saturday), my rash was up and down both of my boobs, in my cleavage, along my bra line in the back and all over my neck, into my hairline. It was PAINFUL, burning, itching. I was insanely irritable. It looked horrific.”

One Reddit user, ketoko, said they had been wearing gloves to bed to prevent from scratching throughout the night. “I’m starting to develop scabs and blisters and it just hurts really bad,” ketoko said in a post.

Even once the rash subsided, some found that the marks stuck around for good.

“I spent years trying to overcome insecurities. I decided to try keto to finally change for the better,” user donut-peach said, “but now I have a totally different problem that prevents me from showing my skin … it was spreading too rapidly and left behind discoloration. I now am off keto and have hideous marks all over my chest, shoulders, neck, and back.”

The condition is called prurigo pigmentosa. Existing scientific literature shows a link between ketosis and the rash, though studies show other intervening factors (such as sunlight, extreme heat, allergens and sweating) may worsen the condition. Going on the keto diet is not the only way humans can enter ketosis. Conditions such as diabetes mellitus and anorexia nervosa can also lead to this state, as well as fasting and dieting.

A 2018 case study showed that prurigo pigmentosa can only really be treated by eating a lot more carbs — i.e., quitting keto. Other options, such as topical creams and anti-inflammatory drugs, may temporarily relieve symptoms — but if you don’t quit the diet, the rash is likely to return in full force once you stop using them. “There is no dietary treatment option to successfully treat the rash without sacrificing ketosis,” the study reads. And once the rash disappears, there is no known method of getting rid of the dark brown spots prurigo pigmentosa leaves behind.

Not everyone on the keto diet experiences this symptom. But you should know if there are serious risks to a diet before you try it.
